Genesis Football Academy

Players participating in the Genesis Football Clinic & Academy program are exercising a privilege afforded them by GENESIS in pursuit of regional, national, and international recognition as youth football players. These players must exhibit the maturity to be successful in this pursuit. Thus, the following guidelines and rules shall apply in all activities with Genesis Football Clinic & Academy:

1. Players are expected to conduct themselves at all times in a manner, which is in keeping with representing GENESIS, and will not bring discredit upon the academy.
2. When travelling with GENESIS each player is expected to dress appropriately as befits representing GENESIS as directed by the Coaching and Management Staff.
3. Respect for property of others, adherence to the rules and guidelines specified here or by the Coaching Staff and observance of Jamaican laws are required for participation in this programme.
4. Irresponsible and disrespectful behavior is cause for dismissal from the programme for the remainder of term.
5. Failure to comply with any and all tournament or team rules (curfew attendance, dress code, schedules, etc.) may be cause for disciplinary action. Persistent failure will be cause for dismissal from the programme for the remainder of this programme and could affect a player's future participation.
6. Negative comments during football clinics and games are strongly discouraged.
7. Fighting, use of abusive or obscene language, stealing, teasing are prohibited.

I will conduct myself in a manner that represents the facilities, other players, referees, and the Coaching, volunteer and Administrative staff of Genesis Football Clinic & Academy while I am participating in the Genesis Football Clinic & Academy. If dismissed from the program or an event occurs while I am travelling with the Genesis Football Clinic & Academy, I may be sent home immediately at my parent's cost by whatever means is most convenient for GENESIS Administrators. No reimbursement of any program fees will be made.

As a Parent/Guardian I agree to abide by the following:

1. I will do my best to make football fun for my child(ren)/ward(s).
2. Support and encourage my child's experience and play on the field.
3. I will remember that the game is for children not adults.
4. I will encourage good sportsmanship and self discipline by demonstrating positive support for all players, coaches and officials at every game, practice through example.
5. Support the coach, manager and the team.
6. Respect officials and accept their decisions.
7. Familiarize myself with the laws of the game.
8. I will support a sports environment for my child that is free of drugs, tobacco, alcohol and abusive language, and I will refrain from their use at all Youth Sports.
9. I will ask my child to treat other players, coaches, fans and officials with respect regardless of their race, sex, creed or ability.
10. I understand that the academy management/coach has the right to remove parents/spectators from the sideline.
11. I will demonstrate positive sideline behavior and never "COACH" from the sidelines.
12. I will ensure my child is available for all practices and games.
13. I will discuss my child ONLY with my coach, not with others, only at a time mutually agreed with the coach. Never prior to, during or after the game. Discuss any and all unresolved issues with the Academy director.
14. I understand that, at the U12 and younger age groups, game playing, time, and position rotations is equitable (but not equal) and, in each case, is at the discretion of the coach. I understand that the coach may handle tournaments and other special events differently than regular season games.
15. I understand that, at the U13 and a above age groups game playing time and positions in any one game is at the discretion of the coach. I understand that the coach may handle tournaments, showcases and other special events differently than regular season games, and that may include no play time for my child in a particular game. I will abide by the "24 hour rule" - requiring that I not contact the coach either in person or via other means within 24 hours after a game to discuss issues.
16. In the event I feel the need to discuss a situation, I understand the proper communication chain is to raise the issue directly with the coach first.
17. In the event I am deemed to have broken this Code of Conduct, I understand that the Academy may levy penalties, including, but not limited to, suspensions of spectator rights at future games or practices, suspension of my child's right to play in future games or practice with the team, or dismissal from the Academy. This will include any relatives and/or friends of parents attending any Academy event.

Waiver of Liability and Release Form

Football is a physical, contact, sport that involves the risk of injury. I assume all risks and hazards associated with my child's/ward's participation in the sport. S/he is in proper physical condition to participate in football practices and games and have no illness, disease or existing injury or physical defect that would be aggravated by his/her participation. I further acknowledge that this risk may involve loss or damage to him/her or his/her property, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of immediate emergency medical care.

Genesis Football Clinic & Academy does not have personal injury insurance that covers my child's/ward's participation. Under any condition, I am responsible for any and all medical expenses arising from my child's/ward's participation in Genesis Football Clinic & Academy programmes. I have the right and responsibility to inspect the equipment and facilities prior to events/clinics and, if I believe that anything may be unsafe, I will refuse participation of my child/ward. Allowing my child/ward to participate is my consent.

I authorize my photograph, picture or likeness, voice, data and personal information (such as address, contact information, D.O.B) or that of my child/ward to appear in any documentary, promotion (including advertising), television, video, social media or radio coverage of Genesis Football Clinic & Academy programs, without compensation.

I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, the Jamaica Football Federation (JFF), the Manchester Football Association (MFA), Genesis Football Clinic & Academy, the league/tournament, their associated directors, administrators, officers, managers, employees, coaches, trainers volunteers, sponsors and advertisers, and other agents, estates or executors, from any and all liability incurred in the conduct of, and my child/ward's participation in, Genesis Football Clinic & Academy programs. This includes owners, lessors, and lessees of premises, municipalities, government agencies, successors, heirs, and assigns.

I have completely read this document and fully understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily. My signature attests to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, successors, and assigns.

As the parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named below. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor in the execution of this Waiver of Liability and Release.

Medical Release Form

Genesis Football Clinic & Academy is committed to you and your family. We take child safety and protection seriously and do all we can to ensure the safety and protection of your children. Inherent in sport however is the possibility of injury. Please read through and sign the waiver below;

I, the undersigned hereby certify that I am the parent/guardian of the player who is registering for Genesis Football Clinic and Academy Football Programmes. If I cannot be reached, I give permission for emergency treatment, emergency transportation, hospitalization, anesthesia, or injection, and will be responsible for the bills of same. My authorization does not include major surgery, unless life-threatening, and only then when the medical opinion of two licensed physicians or dentists concur in that treatment.

I, the undersigned, my heirs, executors and administrators waive, release and forever discharge Genesis Football Clinic & Academy and its staff, coaches, coordinators, employees & volunteers from all rights claims for damages, injury or loss to person or property which may be sustained or occur during participation in football clinics.

Note: All medical conditions, allergies, and current medications submitted in Step 1 will be kept strictly confidential by Genesis Football Academy administration and coaching staff.
